Camps and Conferences Laboratory Safety Questionnaire

Please review the WFU Conferences and Summer Camps Safety Guidelines and Agreement and complete certification confirming you understand the responsibilities and requirements.

These guidelines have been established to protect the health and well-being of persons under the age of 18 who are not enrolled students at Wake Forest University but participating in University-sponsored programs, or in programs operated by outside entities that are held in University facilities.

Permission Forms are to be completed by the WFU host and signed by visiting minor and parent/guardian.
